Commercial Health plans
While Naveris does have some commercial coverage, we are actively pursuing coverage with most major health plans.
We welcome all insurance plans but coverage does vary by health care plan and health care plan type.
Medicare
NavDx testing is covered under Medicare when test-specific medical necessity criteria are met, so eligible fee-for-service beneficiaries typically have $0 out-of-pocket cost.

Our Patient Insurance Support Team can help evaluate your situation and options, including our Naveris Cares financial assistance program.
Financial AssistanceCoverage and Reimbursement Support
Naveris is committed to supporting coverage and reimbursement for our NavDx test and providing financial assistance to those who qualify.
Our Patient Insurance Support Team supports this process by:
- Verifying the correct insurance
- Submitting prior authorizations when possible
- Submitting claims and appeals, when needed, to your health insurance plan
- Helping interpret the explanation of benefits (EOB) that you will receive from your insurance plan
- Administering financial assistance support programs

Billing Process
Your Doctor
Orders a Test
We check your insurance coverage and when necessary, we will work with your doctor's office to submit prior authorization to your insurance company.
Once Testing is Complete, we Bill Your Insurance
Your insurance company will send you an Explanation of Benefits (EOB) once they review and make a decision about your test.
An EOB is not a bill.
Your insurance company will determine if you owe any out-of-pocket costs and this should be detailed in the EOB.
Appeal a Denial
on Your Behalf
If your claim is denied, we will attempt to appeal that decision. We may need your help with the appeal with a signed consent or “authorized representative form”. If you receive one, please complete and send back to Naveris.
Appeals processes can sometimes take several months. You may receive multiple EOBs and coverage status letters from your insurance provider during this process.
Patient
Responsibility
If your test is covered by your health plan, you may be responsible for any out of pocket expense due to patient responsibility (copay, deductible, coinsurance) and determined by your health plan. Naveris will send you an invoice if that occurs. Please pay either by mail or by the link to the online payment portal.
Financial
Assistance

Naveris is committed to ensuring the cost of NavDx testing is not a barrier. You are only financially responsible for your insurance plan-associated costs for the NavDx test, such as coinsurance or deductibles.
Naveris Cares is a patient-assistance program that helps eligible patients access NavDx testing by covering some or all out-of-pocket costs—such as coinsurance and deductibles—so that cost is never a barrier to care. Eligibility for Naveris Cares is determined case-by-case based on each patient's financial situation, prioritizing those experiencing financial hardship.
